- Complete the information request form below and information will be mailed to you usually within 3-5 business days.

- Total Hours Appeal and Advisor Supplement Form
Do not submit this form unless you have have been suspended from Financial Aid for too many credits and have not already appealed. This appeal will not affect your tuition surchage, you must contact the Registration office about an appeal.

- PLUS (Parent Loan) Credit Pre-Approval Request
You may use this form to request a credit check from UHEAA (by fax) before completing a PLUS application. - Selective Service Exemption Appeal
