Questions and Feedback Forms Explained
Information Requests
Utah State University Registrar
This should be used to contact the University Registrar. This can be used to provide positive feedback about experiences at the Registrar's Office, suggestions how to improve procedures, etc.
Registrar's Help Desk
Use this form to inquire about information related to registration, tuition payment, and how to access your student information. Also a helpful resource to indicate a problem using the ACCESS student system.
PIN Problems
This form should be used to resolve PIN problems. Please allow for 1 business day for your request to be processed and completed. An e-mail will be sent to the account holder explaining further information.
Apply for Graduation Online
This form is for those students who wish to apply online for graduation.
Records
Use this form to receive further instruction on learning the process necessary to access or make changes to student history.
Staff and Faculty INB and ACCESS Requests (Security Process)
To be used by Staff, Faculty, Advisors, and Administrators to follow up on their submitted documentation regarding this information. Please remember from the time the form is submitted it may take 3 - 5 business days to process.
Verifications
If you need to verify your student status for various reasons, you can use this form to receive the information necessary to accomplish this.
Montgomery GI Bill
If you are looking for information regarding the Montgomery GI Bill, you can use this form to receive further information.
Leave of Absence
Use this form to be further advised of the required process to follow if you are planning to take a leave of absence.
Perkins Loan
Used to find out more information regarding all facets of the Perkins Loan including but not limited to Entrance, Exit Interviews, Procedures, and Policies.
Room Reservation/Scheduling
Used to reserve a room or schedule an event. Please note that students must provide faculty sponsor information at time the request is submitted.
Feedback, Concerns, Comments, and Suggestions
Student Banner Issues
Please use this as a resource to let us know about issues concerning Banner with regards to the Student Module (i.e., Registration, Tuition Payments, Records, Graduation, Verifications).
Registrar Webmaster
Use this form to notify the Registrar Webmaster of broken links, updated information, or suggestions concerning the site. It is also the best way to contact the webmaster about any other applicable information.
Faculty, Staff, and Advisors
To be used by Faculty, Staff, and Advisors in reference to information, comments, and concerns relating to official University business and job functions.
