If you are returning from a Leave of Absence please follow the steps listed in order to facilitate your return back to Utah State University.

Welcome back to Utah State University from your Leave of Absence! Because you filed a Leave of Absence prior to your departure, in most cases you will not have to apply for readmission.

USU is pleased to have you back, and to facilitate your return, please review the following steps:

1.Prior to returning, please contact Krystin Deschamps. You may speak with her in person at the RFYE Office in TSC 314. Additionally, you may call her at 435-797-0977. Another option is to complete this electronic form Click Here!

2.You will be asked to verify your local and permanent addresses. You will also be asked if you have attended another institution of higher education.

3.Your registration hold will be cleared, but USU strongly recommends that you choose your courses in consultation with your academic advisor.

4.If you are a parent, spouse, or a guardian representing a student, we welcome your calls. However, in accordance to FERPA (federal privacy laws), please be aware that a Information of Release Form must be on file at the Registrar’s Office in order for any USU representatives to assist you with your student’s return.
